Intake Form

Payment
We require a $50 deposit or prepayment in full for all outcall appointments. Cash or checks are acceptable for further payment in-person. Please select from the following pre-payment options:
You may subtract $15 from the session price if you have your own massage table.
Professional Massage
The massage provided is strictly a therapeutic, non-sexual massage. The session will be terminated upon any illicit or sexually suggestive remarks or advances. A refund will not be issued, and further action may be taken.
Travel
Therapists are available to travel within San Francisco city limits only. Sorry, no exceptions.
Cancellation
All cancellations require 24 hours notice. Deposit will not be refunded for cancellations within 24 hours.
Tardiness
Clients have a 10 minute grace period for their
massage. Anything beyond 10 minutes will be taken off
of the massage's total time. Therapist will wait a total of 20
minutes before leaving the site and the client will be
billed the full amount for the service.
Setup/Takedown
Your therapist will arrive 15 minutes prior to the appointment time. Please leave the setup and takedown to us, so you can focus on relaxation and healing.

Outcall Massage Therapy
The ultimate pampering – enjoy a full body massage with no need to fight traffic or look for a parking space! Have the relaxation brought to you, and experience all the benefits of massage in the convenience of your own home or hotel room.
The massage we provide you is custom designed to relax muscles, release tension, and reduce pain. Your outcall table massage will contain a variety of techniques such as Swedish Massage, Deep Tissue, and Myofascial Release.
60 Minutes ~ $100
75 Minutes ~ $115
90 Minutes ~ $130
Your Space
We need a clear space at least 10ft x 8ft for a table massage. The massage should take
place in a quiet room where your
massage will not be interrupted.
For your comfort, the room temperature should be warm (at least 72 degrees is usually preferred). If this is not practical,
you may wish to have a light blanket handy.
If you would like music during your
outcall massage (highly
recommended), please provide a CD
player. We will bring music, but you are always welcome to
incorporate your own music.
